TMS, or transcranial magnetic stimulation, is an FDA-approved method of brain stimulation that is safe, effective, and non-invasive. TMS has been demonstrated to achieve impressive remission rates of patients in clinical trials and has even higher efficacy rates in naturalistic clinical practice settings. Its side effects are mild and have not been proven to worsen symptoms of depression in studies or clinical trials.

Depression is an illness that results from chemical imbalances in the brain, as well as abnormalities in brain structure and neurological connectivity. Antidepressant pharmaceuticals are typically prescribed as the first line of treatment for depression. The FDA approved TMS in 2008, recognizing it as a safe and effective treatment for depression symptoms such as those caused by major depressive disorder, or treatment resistant depression. Patients undergo this form of brain stimulation treatment in a TMS clinic, in the form of TMS sessions.

Tms Is Proven To Be Safe And Effective In Treating Depression

Studies show by and large that TMS is extremely safe and highly effective when used correctly and when the patient is diagnosed correctly with a disorder that TMS is known to treat, like depression.

Additionally, patients meet regularly with their psychiatrist during and after TMS treatment. During this time, your doctor will ask how you are handling treatment and will make adjustments if needed.

While some people may have a negative experience with TMS and may say that TMS ruined my life, we dont recommend that a patient make their decisions based on what happened to a few people. Based on the results from numerous real-world clinical studies, chances are that TMS will work for you.

Both conditions can be successfully treated with a combination of therapy and medications. Antidepressants and antianxiety medications can alleviate the symptoms of depression which overlap with PTSD.

PTSD is often treated with specific types of therapy, most notably cognitive behavioral therapy, or CBT. During a CBT session, a therapist bases the treatment around the trauma that has triggered or contributed to a persons symptoms. The therapy will help the patient identify, understand, and change their thinking and behavior patterns. CBT is focused on helping to alleviate symptoms and give the patient the tools they need to prevent avoidant behavior.

CBT treatments for PTSD tend to occur once per week, for up to 16 weeks. However, the most effective treatment for both PTSD and depression will be tailored to the individual. Some patients will need therapy for longer, while others will make marked improvements in less time. Transcranial magnetic stimulation techniques are also highly effective for alleviating the symptoms of medication-resistant depression and PTSD.

Multiple studies have shown TMS to be an effective maintenance therapy for preventing a depressive relapse. The studies have demonstrated that TMS is effective as a sole maintenance therapy, as well as when used in conjunction with antidepressant drugs. Improved outcomes while using TMS as a sole maintenance therapy were observed with as little as one maintenance session per month.

For the best results, keep pursuing these options even if depression treatment helps you feel better. TMS therapy for depression can be very effective, but its still not a treatment for anxiety and therefore isnt a panacea.

Can Tms Make You Worse

There is no evidence that Transcranial Magnetic Stimulation can worsen symptoms of any condition the treatment is often prescribed for. However, it is possible that people with conditions TMS is not a treatment for, may experience negative effects. Clinical studies of schizophrenia and bipolar disorder show mixed results. TMS seems to improve some of the negative symptoms of schizophrenia, such as hearing voices. Overall, bipolar disorder seems to remain mostly unresponsive to TSM, though there is debate over whether it might make mania symptoms worse. More research is needed to reach a valid conclusion.

Who Is A Good Candidate For Tms

Depression is a complicated condition, and it can be difficult to treat. Usually, the first-line treatment for depression is a combination of medication and therapy. If a medication does not work for an individual, a doctor may change the dosage, type, or both several times.

However, in some cases, this still does not relieve the symptoms, and doctors will determine that the person has treatment-resistant depression.

People who have treatment-resistant depression or cannot take medication for other reasons are good candidates for TMS.

It is not safe for a person to receive TMS if they have any of the following implants in their head:

  • facial tattoos with magnetic ink
  • metallic eye or ear implants
  • bullet or shrapnel fragments

Is Tms Covered Under My Insurance

TMS is not currently covered under UNC Charity Care or Medicaid. Theta Burst Stimulation is currently not covered by most insurances. Medicare and many private insurances do cover TMS specifically for Major Depressive Disorder. Insurance companies that provide reimbursement for TMS generally require a recent failed trial of evidence-based individual psychotherapy as well as four prior medication trials of adequate dose and duration or with evidence of intolerable adverse side effects. Most insurances will require you to engage in a TMS Consultation with one of our psychiatrists to determine your appropriateness and eligibility for TMS at which point we then work with our pre-arrival team to complete a prior authorization form with your insurance.

Exact Efficacy Of Tms Therapy

Although the exact efficacy of TMS is not known, as it is still a fledgling practice, some studies have shown that it does demonstrate consistency in treating patients with depression. However, though it seems less likely to assist with other psychiatric disorders.

Part of its efficacy may lie in the time commitment required for treatment reportedly, many depression patients stop using therapeutic techniques as early as one month after diagnosis, and TMS requires a minimum of 4-6 weeks to conclude treatment. The consistency of treatment, then, may be a significant contributor to its use as a depression treatment.

How Tms Treatment Works

Since the 1980s, TMS treatments have been used to treat depression and anxiety by enhancing activity in areas of the brain, usually the dorsolateral prefrontal cortex . During a TMS session, a doctor uses a magnetic coil to send alternating pulses to neurons to stimulate brain activity. A noninvasive and nonconvulsive alternative to electroconvulsive therapy for medication-resistant depression, TMS is performed usually in 20 minute sessions, where patients remain awake and are able to return to work or their regular routines immediately following the sessions.

Depression is broken down into several different clinical subtypes. Psychotic depression, although rare, is incredibly dangerous and severe. For Major Depressive Disorder , the most common side effects observed in our multicenter randomized clinical trial during treatment include:

  • Site discomfort .

The following side effects were not significantly different in the treatment vs. sham groups, and therefore may not be specifically caused by Deep TMS treatment:

  • Headaches . Such headaches tend to get better or go away completely with successive treatments, and can be alleviated using over-the-counter medication.
