Inclusion Criteria And Exclusion Criteria

Generating clinical evidence for a medical device to treat anxiety and depression

Patients FCs were eligible if they met the following criteria:

patients were pathologically diagnosed with cancer and had underwent medical treatments for one or more cycles

FCs were at least 18 years of age

they could understand and fill in the questionnaires truly and correctly

they took care of patients at home and/or in the hospital

FCs voluntarily participated in this study and provided informed written consent.

Patients FCs were not suitable for this study for the following reasons:

FCs had psychiatric conditions or intellectual difficulties.

In this proposal the investigators will use an accelerated TMS protocol that concentrates the magnetic stimulation that would usually occur over 6 weeks into 10 treatment sessions per days, for 5 consecutive days in patient with treatment-refractory depression. This protocol will build on a previously published study demonstrating clinical efficacy of intermittent theta-burst stimulation on left dorsolateral prefrontal cortex in a treatment refractory population.

Lessons Learned From Clinical Trials Investigating Minor Depressive Disorder

Another concern highlighted by investigations of minor depression is the lack of objective measures of either functional or quality of life impairment. This problem is also true for most studies of most psychiatric disorders. Thus, inspite of the fact that the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition requires functional impairment or quality of life impairment to be present in order for a diagnosis of the syndrome to be made, there have been few efforts to establish some type of criteria for quality of life or functional impairment with these disorders.21 It has been shown in primary care studies that many people who seem to meet criteria for psychiatric syndromes have spontaneous remissions when followed longitudinally. This may well reflect the inclusion of individuals who, because of life stress, have a particular series of signs and symptoms, but in actual fact do not have the pathology associated with a lifelong syndrome. As would be expected, the result of not paying attention to these challenges when designing clinical trials is that the trials tend to be uninformative, if not misleading.

The primary objective will be to study changes in putamen structural connectivity in healthy teens with meditation training. Hypothesis: Putamen structural node strength will increase in the training group compared to controls. R61 Go/No-Go Criteria. Detect an effect size in changes of the primary mechanistic outcome by the described meditation training in 100 healthy adolescents that are 14-18 years old and retain at least 80% of randomized participants for primary outcome measurement at the end of the study regardless of adherence to the intervention. The secondary objective will be to study changes in emotional problems in healthy teens with meditation training. Hypothesis: There will be a significant decrease in emotional problems measured with the Strengths and Difficulties Questionnaire in the training group compared to controls. Design and Outcomes: The current research study design will utilize an individually randomized group treatment, open-label, waitlist-controlled clinical trial to test the efficacy and safety of the investigator’s innovative mindfulness meditation intervention on the primary outcome and secondary outcome in healthy adolescents between the ages of 14 to 18 years old.

The purpose of this study is to pilot test the feasibility, acceptability, and preliminary efficacy of healthcare provider referrals to a tax filing app within parent-child health programs to test whether such referrals can increase receipt of tax credits among low-income parents. The study will use a single-group, pre/post test design with a sample of approximately 100 women who have a child under 6 years of age. Participants will be recruited from parental-child health programs and clinics in Los Angeles and will complete surveys at baseline, immediately after tax filing season, and six months after tax filing season to assess 1) frequency of tax filing after referral , 2) the acceptability of the tax filing app from the perspective of users , and 3) pre/posttest changes to parent and child health, child development, and healthcare utilization measures for users .

How Is Depression Treated

Patients with depression usually have a positive response to treatment. However, it is important to remember that not all patients react in the same way to the same treatments. Therefore, it is possible that you will need to try different alternatives before finding a treatment that works for you. Depression is usually treated with a combination of medications and therapy.

Antidepressants are meant to regulate the brain chemistry in order to relieve depression. Although some improvement might be seen after a couple of weeks into the treatment, their full effect isnt usually apparent until 1-3 months taking the medication, and they are usually prescribed for at least 6 months.

Antidepressants include:

  • Selective serotonin reuptake inhibitors : citalopram, escitalopram, fluoxetine, sertraline, paroxetine, vortioxetine amongst others.
  • Serotonin/norepinephrine reuptake inhibitors : venlafaxine, duloxetine, levomilnacipran, levomilnacipram , amongst others.

Repetitive Transcranial magnetic stimulation is an FDA-approved treatment for depression that involves brief magnetic stimulation pulses on the dorsolateral prefrontal cortex brain region. The ultimate goal of this treatment is to increase excitability and long-term plasticity in DLPFC, a brain region shown to be hypo-active in depression. Unfortunately, rTMS only has low to moderate efficacy remission rates for patients range from ~15-30% in large randomized controlled trials. The focus of this research is to develop a next-generation rTMS protocol that is guided by the basic principles underlying brain plasticity, in order to improve the efficacy of rTMS for the treatment of depression. Specifically, in this study the investigators will test rTMS paired with a depression-relevant cognitive state of internal attention.

The purpose of this study is to evaluate the efficacy and mediators of change in Positive Affect Treatment, a psychotherapy specifically aimed at enhancing reward sensitivity in individuals with low positive affect in the context of depression or anxiety. Target enrollment is 100 male and female participants with low positive affect and depression or anxiety and impaired functioning, between the ages of 18 and 65 years, who will be randomized to either Positive Affect Treatment or Negative Affect Treatment . Participants will complete laboratory tests, psychiatric assessments, and self-report questionnaires as part of the study. The total length of participation is around 5 months.

Mindfulness Meditation Neuroimaging Study For Undergraduates

This study is designed to test the effectiveness of mindfulness meditation intervention classes on psychosocial health outcomes. Additionally, the investigators would like to examine changes in the brain that might occur following the mindfulness meditation intervention. The investigators are particularly interested in changes in brain activity that are correlated with changes in inflammation-related markers in the blood. The nervous system and immune system are closely connected, and both are influenced by mindfulness. However, it is unclear whether changes in neural activity are linked with changes in inflammation. A compelling feature of mindfulness interventions is their potential for reducing inflammatory activity however, this has not been examined in college students. In addition to measuring psychosocial outcomes, the investigators will employ sophisticated, vertically-integrated measures of inflammatory biology that allow the study team to probe intervention effects on circulating markers of inflammation. Thus, the investigators intend to recruit 60 undergraduate students and will randomize them into either a 6-week standardized mindfulness intervention or to a wait-list control group. Participants will complete brain scans, provide blood samples for immune analysis, and complete questionnaires at pre- and post-intervention assessments

Bipolar Efficacy Biomarkers For Rtms

The research study is being conducted to test whether using high dose spaced theta-burst rTMS produces a significant reduction in depressive symptoms compared with sham. This project will recruit patients aged 18-70 with symptoms of bipolar depression who have failed after at least two prior treatments. The null hypothesis is that there will be no difference in reductions in depressive symptoms by the end of a five-day treatment period. The alternative hypothesis is that, compared with sham, active TMS will result in a greater reduction in depressive symptoms by the end of the treatment period. To facilitate the development of rTMS protocols there is a need for biomarkers that are sensitive to BPD symptom severity and clinical improvement. Previously in our lab, investigators developed biomarkers suitable for depression trials, and these biomarkers are very likely to show sensitivity to BPD, since they are associated with brain regions and functions associated with BPD. As a secondary aim, the investigators will try to identify biomarkers in cortical region associated with BPD, and formulate a statistical model that may be able to predict BPD remission after the treatment. this study will lead to development of new brain stimulation treatment protocols and biomarkers, will aid in treatment selection, and eventually lead to better clinical outcome for patients suffering from BPD.

This study will examine the effects of brain stimulation on pain symptoms associated with Major depressive disorder. This study will enroll 69 Subjects. Study subjects will be asked to complete surveys about their mood and well-being, 2 blood draws, 2 MRIs, 3 electroencephalograms, and receive 30 treatments of blinded transcranial magnetic stimulation. There is no control group as all subjects will receive some form of active treatment. Subjects are required to participate in 30-33 study visits and volunteer 40 hours of their time. Compensation for this study is $150 for completing all study activities.

The exploratory Phase 2 clinical trial is a U.S. multi-center, randomized, double-blind, placebo-controlled study intended to evaluate efficacy, safety and tolerability of PH94B administered four times per day over four weeks for the treatment adjustment disorder with anxiety symptoms in adults. The primary endpoint is the change from baseline in anxiety level as measured by the Hamilton Anxiety Rating Scale at the end of Week 4 of treatment with PH94B or placebo.
