Mood disorders are relatively common in adults, with depression and bipolar disorder being the most common. Approximately 7% of adults in the United States have depression, while about 2.8% have bipolar disorder.

Mood disorders are commonly seen in children and adolescents approximately 15% have any mood disorder.

What Is A Mood Disorder

A mood disorder is a mental health condition that primarily affects your emotional state. Its a disorder in which you experience long periods of extreme happiness, extreme sadness or both. Certain mood disorders involve other persistent emotions, such as anger and irritability.

Its normal for your mood to change, depending on the situation. However, for a mood disorder diagnosis, symptoms must be present for several weeks or longer. Mood disorders can cause changes in your behavior and can affect your ability to perform routine activities, such as work or school.

Two of the most common mood disorders are depression and bipolar disorder.

When To See A Doctor

Depression with psychosis is a very serious medical condition that requires immediate medical attention.

As a result of psychosis, the risk of suicide is significantly higher in people with psychotic depression as compared to people with nonpsychotic depression.

If a person is or knows someone who is experiencing any of the symptoms listed above, they should speak to a healthcare provider immediately.

If someone is having thoughts of harming themselves or others, they can either call 911 or go a hospital emergency room.

In the U.S., they can also reach the National Suicide Prevention Lifeline at 1-800-273-8255. Trained staff are available 24 hours a day, 7 days a week.

There is some debate around the best treatment process for psychotic depression, particularly regarding first- and second-line treatments.

First-line treatment usually involves a combination of antidepressant and antipsychotic medications, or monotherapy, which refers to the use of either antidepressants or antipsychotics alone.

Doctors typically reserve electroconvulsive therapy for second-line treatment, and specialists may use it if different medications have not helped to alleviate symptoms.

Psychotherapy, or talking therapies, can also help people, as a supplemental treatment for depression with psychosis.

Depressive Symptoms Associated With Aggression

The authors review the association between depression and violence, and the role that anger and emotional dysregulation play.

Violence is not usually considered to be related to depression, yet findings suggest an association between violent behavior and depression or depressive symptoms in many different disorders. A Swedish study compared the criminal records of 47,158 depressed individuals with the records of 898,454 people with no history of depression matched by age and sex.1 Those in the depressed group were approximately 3 times more likely than the general population to commit violent crimes, such as homicide, attempted homicide, aggravated assault, or robbery.

This association was present even when previous histories of violence, self-harm, psychosis, and substance use were taken into consideration. Furthermore, the risk of violent crime significantly increased in individuals with more depressive symptoms.

In dementia, as well, patients with depressive symptoms are more likely to exhibit physical aggression.6 And in bipolar disorder, increased violence is not limited to the manic phase but is also present in bipolar depression.7

When we assess the risk of violence in people with depressive symptoms, we must consider the specific disorder in which these symptoms exist.

Anyone can have problems with anger or irritability, but those problems are more common for people who live with depression or bipolar disorder. Irritability or anger outbursts are common symptoms of mania, and some people have problems with irritability when they are more depressed. Do your problems with anger tend to go along with other symptoms of mania or symptoms of depression ?

If you are taking medication for depression or bipolar disorder, be sure to tell your doctor about problems with anger or losing control and about how those problems relate to other mood symptoms. If anger problems seem to be symptoms of depression or mania, then your doctor may recommend some adjustment in medication.

Whether or not anger problems are related to depression or mania, there are specific steps you can take to better manage irritability or control anger outbursts. These include:

  • Learning to recognize the external situations that tend to make you more irritable or angry.
  • Planning ahead for managing those high-risk situations by either avoiding them or preparing for them.
  • Learning to recognize the internal warning signs that indicate you are more irritable or angry.
  • Identifying specific things you can do when you notice those internal warning signs. Those might include things you can say to yourself, physical relaxation , or actions you can take to interrupt the anger .

How Prevalent Is Anger In Depression And What Does It Look Like

Major depressive disorder is a common mood disorder that affected more than 17 million U.S. adults in 2017. The prevalence of a major depressive episode was highest among adults ages 18 to 25 and higher for women than men .

The Diagnostic and Statistical Manual of Mental Disorders, 5th edition states that an individual must exhibit specific symptoms to be diagnosed with major depression, one of which must be either depressed mood and or loss of interest or pleasure in life activities for at least two weeks. The DSM-5 also requires the majority of the following symptoms that cause clinically significant impairment in social, work or other important areas of functioning almost every day:

  • Significant unintentional weight loss or gain.
  • Insomnia or sleeping too much.
  • Feelings of worthlessness or excessive guilt.
  • Agitation or psychomotor retardation.
  • Fatigue or loss of energy.
  • Diminished ability to think or concentrate, or indecisiveness.
  • Recurrent thoughts of death.

Though anger isnt listed by the DSM-5 as one of the diagnostic criteria for adults, irritability is a key diagnostic factor for children and adolescents. In an interview with NPR about anger and depression, Dr. Maurizio Fava, a psychiatrist at Massachusetts General Hospital and a professor at Harvard Medical School, questioned whether the DSM-5 is getting it right.

Why would someone who happens to be irritable and angry when depressed as an adolescent suddenly stop being angry at age 18? he said.

How Can Someone Deal With Their Anger Issues And Depression

Medication is useful for addressing and treating the symptoms of depression, but medication alone cannot effectively treat and alleviate maladaptive anger. In depressed patients where anger plays a significant role in their disorder, itâs crucial that maladaptive anger is addressed in a clinical setting. Treating maladaptive anger, and turning those maladaptive traits into adaptive traits can greatly reduce the negative emotional consequences of rage turned inward.

One of the most effective ways of treating anger and depression is with a combination of pharmacological means and behavioral therapies. Cognitive behavioral therapy, or CBT, addresses anger issues in a controlled environment. A trained therapist can help patients recognize their anger and cope with it in adaptive ways to effect positive, lasting changes in the patientâs life. Also, CBT is a well-known technique for managing the symptoms of depression. Patients who suffer from anger issues and major depressive disorder can make significant gains when combining drug treatment therapies with CBT.

Although anger is a significant contributor, and also a symptom of depression itself, patients who address their anger issues in therapy will experience a lessening of depressive symptoms and a shortening of the duration of the episode.

Irritability Anger Indicators Of Complex Severe Depression

Symptoms of irritability and anger during a major depressive episode appear to be clinical markers for a significantly more complex, chronic, and severe form of major depressive disorder, a new study indicates.

Results from the longitudinal observational investigation of patients with unipolar MDEs showed that those with current overt irritability/anger were significantly more likely to have increased depressive severity, longer duration of the index MDE, poorer impulse control, and a more chronic and severe long-term course of illness.

Overt irritability and anger were also associated with higher rates of lifetime comorbid substance abuse and anxiety disorder, more antisocial and personality disorders, greater psychosocial impairment, reduced life satisfaction, and a higher rate of bipolar II disorder in relatives.

The study findings strongly suggest that “concurrent anger/irritability symptoms are important indicators of increased severity, chronicity, and complexity of unipolar major depression,” the authors, led by Lewis L. Judd, MD, University of California, San Diego, write.

The study was September 11 in JAMA Psychiatry.

The investigators examined the prevalence of overtly expressed irritability/anger in MDE and its association with illness presentation and long-term course in 536 patients followed for up to 31 years.

Favas study indicated that anger attacks disappeared in 53% to 71% of depressed patients who used medication.

What Does Irritability Look Like When Its Part Of Depression

There are many things that can prompt irritability and anger in young people. What are the signs that these negative moods might be associated with depression?
