Psychosis describes a disconnect from reality, a state of mind where you have trouble recognizing whats real and what isnt. In terms of bipolar disorder, psychosis mainly refers to hallucinations or delusions.

Other key symptoms of psychosis, such as changes in sleep patterns or problems with concentration, can seem very similar to those that characterize mood episodes.

Bipolar psychosis might involve:

These symptoms can be mood congruent, meaning theyre consistent with your other symptoms. They can also be mood incongruent, or inconsistent with your other symptoms. Believing you have superpowers during an episode of depression, for example, would be considered mood-incongruent psychosis.

Psychosis can happen during both manic and depressive episodes. An episode of hypomania that involves psychosis automatically meets the criteria for mania.

How Is Psychotic Depression Different From Major Or Clinical Nonpsychotic Depression

According to the National Institute of Mental Health, a person who is psychotic is out of touch with reality. People with psychosis may hear voices. Or they may have strange and illogical ideas. For example, they may think that others can hear their thoughts or are trying to harm them. Or they might think they are possessed by the devil or are wanted by the police for having committed a crime that they really did not commit.

People with psychotic depression may get angry for no apparent reason. Or they may spend a lot of time by themselves or in bed, sleeping during the day and staying awake at night. A person with psychotic depression may neglect appearance by not bathing or changing clothes. Or that person may be hard to talk to. Perhaps they barely talk or else says things that make no sense.

People with other mental illnesses, such as schizophrenia, also experience psychosis. But those with psychotic depression usually have delusions or hallucinations that are consistent with themes about depression , whereas psychotic symptoms in schizophrenia are more often bizarre or implausible and have no obvious connection to a mood state . People with psychotic depression also may be humiliated or ashamed of the thoughts and try to hide them. Doing so makes this type of depression very difficult to diagnose.

Does Treatment For Psychotic Depression Always Work

Treatment for psychotic depression is very effective. People are able to recover, usually within several months. But continual medical follow-up may be necessary. If the medications do not work to end the psychosis and depression, sometimes electroconvulsive therapy is used. It’s important for the patient to work with the doctor to find the most effective drugs with the least side effects. Because psychotic depression is quite serious, the risk of suicide is also great.

Is There A Difference Between Schizophrenia And Psychotic Depression

There are defined differences between major depression with psychotic features, schizophrenia, and bipolar disorder. Unfortunately, these conditions are sometimes categorized together which is unhelpful for those living with any of these disorders.

If someone experiences predominantly psychosis, without such a prominently depressed mood, its likely they are experiencing schizophrenia. Individuals with schizophrenia will experience hallucinations and delusions that persevere. They may also have lowered cognition, numbed feelings, and loss of motivation.

Its more common for schizophrenia to develop when the individual is in their teens or early 20s, although psychotic depression can develop at any age and, as aforementioned, can even be seen for the first time in later life. Schizophrenia tends to be more long-lasting rather than episodic.

Bipolar disorder is a different condition, which can be categorized into three different types. This is a condition that results in severe changes to mood, energy, activity levels, and concentration, impacting the ability to regulate the individuals life.

Its important when we are talking about mental health to make clear distinctions between conditions, this can help us to understand treatment better, reduce stigma, and improve the lives of those affected.

What Are The Signs And Symptoms

Major depression symptoms vary from person to person. To receive a diagnosis of major depressive disorder, some of these signs and symptoms must be present for at least two weeks. Anyone who has questions should consult their doctor.

  • Continued feelings of sadness, hopelessness, pessimism, emptiness
  • Fatigue, lack of energy

How Psychotic Depression Differs From Schizophrenia

If the person has pure psychosis, without much depression, it is more likely to be schizophrenia, says Amit Anand, MD, professor and Vice Chair for Research at the Center for Behavioral Health at the Cleveland Clinic. And with schizophrenia, the depression is usually not predominant and the person has hallucinations and delusions that dont go away. They also have decreased thinking, feelings and motivation.

Schizophrenia tends to make an appearance for the first time when the patient is in their late teens or early 20s while individuals can have psychotic depression at any age, Dr. Rothschild says. Schizophrenia is more long-lasting and is not episodic, he explains. The delusions tend to be more bizarre, although we dont make the diagnosis based on the type of delusion.

Depression with psychosis is a very serious medical condition that requires immediate medical attention.

As a result of psychosis, the risk of suicide is significantly higher in people with psychotic depression as compared to people with nonpsychotic depression.

If a person is or knows someone who is experiencing any of the symptoms listed above, they should speak to a healthcare provider immediately.

If someone is having thoughts of harming themselves or others, they can either call 911 or go a hospital emergency room.

There is some debate around the best treatment process for psychotic depression, particularly regarding first- and second-line treatments.

Doctors typically reserve electroconvulsive therapy for second-line treatment, and specialists may use it if different medications have not helped to alleviate symptoms.

Missed Diagnosis Of Psychotic Depression

Data from the National Institute of Mental Health Study of the Pharmacotherapy of Psychotic Depression 2 indicates that clinicians frequently miss the diagnosis of psychotic depression, in large part, due to a lack of recognition of the psychotic features. In the STOP-PD Study, 27% of 130 diagnoses among a well-characterized sample of patients with a research diagnosis of psychotic depression were initially incorrectly diagnosed. It is likely that the missed diagnosis rate observed in this study was a conservative estimate of the rate in the general clinical population because patients with comorbid conditions such as a history of substance abuse in the past 3 months or unstable medical conditions were excluded. Psychotic depression was most commonly misdiagnosed as major depressive disorder without psychotic features, depression not otherwise specified , or mood disorder NOS. It was quite striking that none of the patients with missed diagnoses were considered to have a psychotic disorder. This finding suggests that the clinicians were missing the psychosis rather than the mood disorder.

Classification Of Psychotic Depression

While the DSM-5 lists psychotic depression as a subtype of major depression, it does not indicate that this type of depression is any more severe than other subtypes. The International Classification of Diseases , on the other hand, classifies psychotic depression as the most severe form of major depressive disorder.

In order to be diagnosed with major depressive disorder with psychotic features, you must experience at least five depressive symptoms for a minimum of two weeks. Such symptoms include low mood, loss of pleasure or interest, irritability, appetite changes, and changes in sleep.

A diagnosis of psychotic depression also includes experiencing symptoms of psychosis, such as paranoia, hallucinations, and delusions, in addition to the depressive symptoms.

How Is Psychotic Depression Diagnosed

People experiencing depression with psychosis dont always seek help on their own. In some cases, it might be a family member or close friend who helps them find a medical or mental health professional who can make a diagnosis.

To make a diagnosis, theyll generally start by asking questions about your mental health, mood, and emotional well-being. They might ask about:

  • fixed beliefs or persistent worries that affect your daily life
  • things you see, hear, or feel that no one else seems to notice
  • problems with sleeping, eating, or going about your daily life
  • your support network and social relationships
  • health concerns
  • other mental health symptoms, like anxiety or mania
  • your personal and family health and mental health history

Psychosis isnt always obvious, even to trained clinicians. Some mental health professionals may not immediately recognize the difference between fixed delusions and rumination, a pattern of looping sad, dark, or unwanted thoughts.

Both delusions and rumination, which is common with depression, can involve:

  • guilt over mistakes you believe youve made
  • perceptions of yourself as a failed partner or parent

Describing all of your feelings, perceptions, and beliefs to your clinician can help them make the right diagnosis.

A diagnosis of major depression also requires that symptoms:

  • last for 2 weeks or longer
  • affect some areas of daily life
  • arent related to substance use or another condition

Specific Combinations Of Medications Studied For The Acute Treatment Of Psychotic Depression

In a recent meta-analysis of antidepressant and antipsychotic trials for the treatment of psychotic depression, Farhani and Correll pointed out that efficacy has been demonstrated only for specific medication combinations and that there is a need for further studies to help elucidate the effectiveness of different combinations of medications. Given this, it is important to keep in mind that the studies discussed below may not be generalizable to all patients with psychotic depression or all combinations of antidepressant and antipsychotic medications. I was able to identify 7 studies in the medical literature that have compared the combination of an antidepressant and an antipsychotic to treatment with antidepressant monotherapy, antipsychotic monotherapy, or placebo in well-designed, double-blind, randomized controlled trials.

Data from the National Institute of Mental Health Study of the Pharmacotherapy of Psychotic Depression indicate that psychiatrists frequently miss the diagnosis of psychotic depression, in large part because they do not recognize the psychotic features. In the STOP-PD study, the diagnosis of psychotic depression was missed 27% of the time, which was likely a conservative estimate because patients with comorbid conditions, such as substance abuse in the past three months, or unstable medical conditions were excluded.

Treatment Implications Of Psychotic Features

Clearly, the presence of delusions and/or hallucinations brings additional, significant challenges to treatment. If a patient indeed has a history of psychosis while depressed, it is essential to inquire about the symptoms each session. It is not unusual for psychotically-depressed patients to require hospitalization, which you, as therapist, may be instrumental in organizing if they begin to present a heightened risk to self or others.

Even if a patient isnt psychotic at the moment, knowing if they have a history of becoming psychotic when depressed is important. At the first sign a depressive episode is setting in, it is a good time to encourage a visit to their psychiatrist to assess for use of an antipsychotic medication to augment their antidepressant, for example. In extreme cases, electroconvulsive therapy , which is explored more fully in an upcoming post in this series on melancholic features, may be necessary . Ideally, we also will incorporate their family/friends by educating them on signs and having the patient agree to let them notify providers of concerns.
