To the speaker, these sentiments are absolute truths his or her sense of identity, a soul-less place to live that is familiar, thus offering a comfortable discomfort, with no exit door. As long as these annihilating beliefs rule your self-image, nothing good can break through. When I ask, Tell me good qualities about yourself, I am initially greeted by silence. Then I hear a halting, Im kind or Im caring.
If the patient gets stuck, I help out: Youre a loving mother. You are a survivor. You are a nurturer You are super considerate. You are reliable As we construct a list, I ask the patient to write down the wonderful attributes and keep repeating them when the toxic wheel of self-hating talk begins. I suggest asking friends and family members to email a list of positive qualities they value in my patient. The next step will be to compile, print out the list and carry it in your wallet like a talisman.

Practice These Coping Skills Every Day
I recommend doing many if not all of the following coping skills and techniques once a day when experiencing depression. Its important to know you probably wont be motivated to do any of them at first because depression frequently saps motivation. In other words, know that its normal to feel unmotivated until youre halfway done.

1. Meaning: Find small ways to be of service to others.

Exercise doesn’t have to be excruciating
When we were kids, my siblings and I would often go down to our Dads exercise lab on the weekends. Dad was a cardiologist, an exercise physiologist, and his laboratory was packed with treadmills and bicycle ergometers. Dad would hook us up to the EKG machines and have us breathe into plastic tubes that measured our O2 consumption as we bicycled or ran up imaginary hills. We grew up indoctrinated with the importance of aerobic exercise to prevent heart disease and hypertension and stroke. It seemed obvious that these principles would extend throughout medicine.
So imagine the disconnect when I began psychiatric training and found that exercise played no role whatsoever in the treatment of depression or other psychiatric disorders. In the early 80s at least, psychiatry was practiced in smoke-filled consultation rooms, and was all about the mind, not the body or the brain.
Now, however, a significant body of data has accumulated to show the benefits of exercise–and other complementary modalities like yoga and mindfulness meditation–which have increasingly become a part of standard practice.

What You Can Do

Sometimes, we get it right the first time, but oftentimes it takes a few tries to find the right combination of treatments that provide the maximum benefits to the patient.
