Tramadol Is An Odd Unpredictable Opioid Scientists Say

Scientists whove studied the curious chemistry of the opioid tramadol use an array of adjectives to describe it: unpredictable, messy, crazy.

Tramadol is unlike most other opioids in that it must pass through the liver to be metabolized into its most potent form. At the same time, it releases another type of drug that acts as an antidepressant because it increases levels of serotonin in the brain, which elevates mood.

But how much opioid and how much antidepressant is released is heavily dependent on users genetics, said Bertha Madras, professor of psychobiology at Harvard Medical School. One person might get a potent dose of opioid, while the next gets nearly none.

Tramadol is most likely not as problematic as some of the big gun opioids like oxycodone and fentanyl and heroin, Madras said. But it certainly can be in certain populations and it certainly can be if its abused by a wider and wider swath of the population.

First Study To Document Real

Previous studies with healthy volunteers have shown effects on blood levels when combining tramadol with these particular antidepressants. However, this is the first study to document the effects of this interaction in a real-world setting with patients.

We knew that there was a theoretical problem, but we didnt know what it meant as far as whats happening to pain control for patients, Mr. Frost says.

What explains the interaction between tramadol and these antidepressants?

Tramadol relies on activation of the CYP2D6 enzyme to give you that pain control, Mr. Frost says. This enzyme can be inhibited by medications that are strong CYP2D6 inhibitors, such as fluoxetine, paroxetine and bupropion.

Acute Anxiety/depression 4 Weeks After Quitting Tramadol

Certain Antidepressants May Reduce Effectiveness of Tramadol
Gerrymoo

Just wondered if anyone could relate to my experience.

Due to a bad back I was taking tramadol 50mg or 2 x 50mg per day for four months. I never really felt well on them – shaky, cold sweats, nausea but after a particularly bad couple of days I couldn’t face taking them cause they made me feel so bad.

I quit cold turkey and never took another but had intense awful withdrawals for a week followed by less intense but manageable withdrawals. 2 weeks later I was suddenly hit by a wall of sudden and extreme nausea and anxiety and feelings of just awfulness. This carried on for weeks and then escalated again into what was bordering in panic attacks which I have never experienced anything like this or anxiety in my life.

I am now just trying to piece it all together and personally I think it’s all related to the quitting of tramadol. What a dreadful experience- any thoughts anyone?

Can You Become Addicted To Tramadol

Because tramadol is an opioid there is always a possibility of addiction.5 While there may have been less stringent rules in the past, numerous studies in countries all over the world have shown that tramadol is not as harmless as it was once marketed. In the past, tramadol was seen as a drug that functioned as a narcotic without being one.6 The thinking behind this was to develop an extremely mild opioid that could be used to replace stronger opioids to lessen the chance of addiction.

Unfortunately, this proved to be hopelessly optimistic. Doctors soon realized that because tramadol was weaker than its counterparts, larger and larger doses would have to be administered to get the same pain relief as before.7 Patients would then build a tolerance to the drug and as time would go on would need more of it. This would occur even as they were supposed to be weaned off it.

What Is Tramadol How Does Tramadol Work

Tramadol is prescribed over other opioid pain medications because it does not directly bind to opioid receptors in the brain like morphine and others. However, comparatively 100 mg of Tramadol equals 10 mg of morphine. The idea that it is a weaker and less addictive opioid is, therefore, misunderstood. The research that has been completed on Tramadol is varied.

Some studies found Tramadol useful for treating anorexia, premature ejaculation, restless leg syndrome, and depression. Because Tramadol works in the same manner as the antidepressant Effexor with selective serotonin reuptake inhibitors , studies on its effectiveness for various diseases have received credible findings.

There is potential for Tramadol to effectively treat anorexia, among many other off-label uses. Because this drug can possibly treat so many medical conditions besides just its standard opioid-based treatment application for pain management, research is on-going in all these areas. In the future, Tramadol may be approved by the FDA to officially treat other conditions and diseases besides just chronic pain. The National Center for Biotechnology Information even explained that Tramadol is used off-label for premature ejaculation and restless leg syndrome, among other medical conditions.

Tramadol May Interact With Other Medications And Cause Negative Effects

Since tramadol is a synthetic opioid, it can negatively interact with other medications, Mayo Clinic notes. For example, if you take certain cold or allergy medications along with tramadol, sedative effects may increase, which can be dangerous.

âTramadol can cause dizziness or drowsiness, and should not be mixed with other medications that can produce a similar effect,â Aaron Sternlicht, LMHC, CASAC, a therapist and co-founder of Family Addiction Specialist in New York, tells WebMD Connect to Care.

The University of Michigan Health notes that, before starting tramadol, you should inform your doctor if you are using any of the following medications:

  • Cold or allergy medicines
  • Medications for heart problems and seizures
  • Medicines for motion sickness, overactive bladder, or irritable bowel syndrome.
  • Sedative medications

Tramadol May Cause Serious Side Effects If Used With Alcohol Or Opioid Medicines

How Does Xanax Work?

âUsing tramadol, or any narcotic pain medication, with other drugs that also cause central nervous system depression such as alprazolam or ETOH can lead to serious side effects including impaired judgment, depressed respiration, coma, and death,â Alan Thomalla, PhD, a licensed psychologist at ABC Resources, tells WebMD Connect to Care.

âIf a person combined tramadol with a similar analgesic opioid agonist such as fentanyl, this effect and subsequent risk would be magnified tenfold,â Thomalla adds.

Pain Associated With Depression

Depression may increase the risk of a headache, chest pain and musculoskeletal pain within the next three years after its diagnosis . However, pain complaints reported by patients with depression may not have a somatic background . Chou et al. argue that patients with depression are often diagnosed with headaches or muscle pain, including lower back pain. Comparing the associations of lower back pain, depression, and anxiety with other non-lowerback pains between orthopedic outpatients with chronic low back pain showed that some other pains, such as a headache, chest pain, and general muscle pain, appeared to be more strongly associated with depression than with the pain intensity of low back pain. The results of a meta-analysis by Bair et al. indicate that the frequency of pain reporting in psychiatric patients varies from 15% to even 100% .

Depression and painful symptoms

Depression is seen in about 50% of patients with chronic pain . Major depression occurs in 53% of patients in pain clinics, 38% in psychiatric clinics, 56% in orthopedic and rheumatology clinics, 85% in dental clinics dealing with craniofacial pain and 13% in gynecological clinics. Patients with at least two pain ailments are 6 to 8 times more likely to develop depression. Patients who have been in pain for over six months have also been reported to suffer from depression four times more often than patients not reporting chronic pain .

Why These Conditions Often Occur Together And How To Treat Them When They Do

Everyone experiences pain at some point, but in people with or anxiety, pain can become particularly intense and hard to treat. People suffering from depression, for example, tend to experience more severe and long-lasting pain than other people.

The overlap of anxiety, depression, and pain is particularly evident in chronic and sometimes disabling pain syndromes such as , , low back pain, headaches, and nerve pain. For example, about two-thirds of patients with irritable bowel syndrome who are referred for follow-up care have symptoms of psychological distress, most often anxiety. About 65% of patients seeking help for depression also report at least one type of pain symptom. Psychiatric disorders not only contribute to pain intensity but also to increased risk of disability.

Researchers once thought the reciprocal relationship between pain, anxiety, and depression resulted mainly from psychological rather than biological factors. Chronic pain is depressing, and likewise major depression may feel physically painful. But as researchers have learned more about how the brain works, and how the nervous system interacts with other parts of the body, they have discovered that pain shares some biological mechanisms with anxiety and depression.

In addition, two neurotransmitters and norepinephrine contribute to pain signaling in the brain and nervous system. They also are implicated in both anxiety and depression.
