Symptoms of depression can occur along a spectrum both in duration and severity. Mild depression can occur during periods of stress but resolve with time and may not require any specific treatment. Moderate to severe depression causes chronic symptoms and usually requires at least one form of treatment, if not multiple treatments.

Generally speaking, severe depression requires some type of treatment to find some relief. Additionally, depression severity can change over time, growing increasingly worse or alternating between mild and severe during the same depressive episode.

If someone has severe depression, they may self-harm, have suicidal thoughts, or be at risk for attempting suicide. If you are severely depressed, help is available to help you manage your symptoms and ensure your safety and well-being.

Know That You Wont Always Feel Alone And Unloved

I know what it feels like to be alone in the world. I grew up moving in and out of foster homes. My mom struggled with paranoid schizophrenia, shock treatments, and powerful anti-psychotic medications my whole life. She called me names, told me she wished she wouldve given me up for adoption. My Canadian half-sister sister wont talk to me I dont have contact with any other family member. My dad lives in Israel with his wife and three adult children, who have 10 children of their own! I didnt even meet my father until I went to Jerusalem when I was 27 years old.

Most of my life I felt like no one cared about me. In my late 20s when I met my dad in Israel I learned how to forgive my parent for not acting like they cared about me. But it wasnt until my 40s that I truly started to come out from under the shadow of feeling unloved and unwanted. It was a long process to heal from the pain of being rejected, abandoned, unloved and unwanted for so long.

Good news! You dont have to live with the pain of feeling like no one cares about you for as long as I did. Are you living at home, or with abusive family members? Read How to Cope With Abuse When You Cant Leave Home.

Dealing With Depression: When No One Cares

Researchers continue to study depression to improve the way this medical condition is diagnosed and treated. For example, NIMH researchers are currently working to understand how and why changes in reproductive hormones trigger mood disorders, including postpartum depression, premenstrual dysphoric disorder, and perimenopausal depression.

NIMH scientists are conducting a large number of research studies with patients and healthy volunteers to better understand why some women are at higher risk than others, and how they can translate these findings into new treatments or new uses of existing treatments.

Pilgrimage Of Desire: A Path Out Of Walking Depression

My lifes work is to help writers and artists recognize their depression and find healing by making their creative work a priority.

One of the ways I do that is by sharing my own story of depression and recovery in a memoir called Pilgrimage of Desire. The book is interspersed with coaching questions and exercises, which help readers take their own steps toward happiness.

As a young adult, I longed to make my mark on the world as a writer. But after university, I got sidetracked by all the demands of ordinary life.

Soon I joined the ranks of the walking depressed. I was working, volunteering, and looking after my family, but I was also desperately sad.

I found the path out of depression by following my desiresto write, to travel, to become a mother and a creativity coach. Eventually I left ordinary life behind. My husband quit his job and we sold our house and headed out on a trip around the world with our kids, aged five and three. I thought Id found my happy ending, but there was more to the story

Do these signs ring true for you? Have you ever been depressed and kept on walking?

I invite you to share your own experiences in the comments. When you do, please be kind to yourself and others. Kindness is the watchword. To keep this space safe and helpful, I remove comments that are unkind or invalidate other peoples experience of depression.

You Cant Just Snap Out Of Depression

Well-meaning friends or family members may try to tell someone with depression to snap out of it, just be positive, or you can be happier if you just try harder. But depression is not a sign of a persons weakness or a character flaw. The truth is that most people who experience depression need treatment to get better.

If you are a friend or family member of a woman with depression, you can offer emotional support, understanding, patience, and encouragement. But never dismiss her feelings. Encourage her to talk to her health care provider, and remind her that, with time and treatment, she can feel better.

What Treatment Should I Be Offered

    Black background

The National Institute of Health and Care Excellence writes guidance on what treatment doctors should offer you. But your doctor does not have to give you these treatments. And the treatments may not be available in your area.

Different treatments may be available in your area. Your doctor might think these suit your symptoms more than the recommended treatments.

NICE recommend that depression is treated in different steps depending on how severe the condition is for you. The steps are as follows.

Step 1: Everyone who may have depression

    Your doctor should offer you:

    • an assessment of your symptoms,
    • support, such as regular appointments in person or by telephone,
    • information on how to deal with your symptoms,
    • monitoring of your symptoms and follow-up, and
    • referral for further assessment and treatment if needed.

Step 2: Mild to moderate depression

    Your doctor may offer you:

    • low-intensity interventions, such as self-help guided by the doctor or computerised cognitive behavioural therapy ,
    • physical activity programmes,
    • group cognitive behavioural therapy ,
    • medication if you have a history of moderate or severe depression, or you have had symptoms for a long time, and
    • referral for further assessment and treatment if needed.

Step 3: Moderate to severe depression, or mild to moderate depression when other treatments havent worked

    Your doctor may suggest:

Step 4: Severe and complex depression or if your life is at risk Your doctor may suggest:

    • medication,

What If My Symptoms Dont Improve

If youre not responding to treatment, you may live with treatment resistant depression. This is when your symptoms have not improved after at least 2 standard treatments. This can also be known as treatment-refractory depression.

There is currently no official criteria used to diagnose treatment resistant depression.

What treatment is available for treatment-resistant depression? There are treatment options for treatment resistant depression. Even if antidepressants have not worked already for you, your doctor may suggest a different antidepressant from a different class.

The new antidepressant you are offered will depend on the first antidepressant you were given.

Sometimes your doctor can prescribe a second type of medication to go with your antidepressant. This can sometimes help the antidepressant work better than it does by itself.

Where antidepressants have not worked, your doctor may suggest talking therapies, ECT or brain stimulation treatments. See the previous section for more information on these.

What is an implanted vagus nerve stimulator, and how is it used in treatment resistant depression? If you live with treatment resistant depression, and youve not responded to other treatments, you may be able ask for an implanted vagus nerve stimulator.

Please speak to your doctor if youre interested in this treatment and for more information. You may be able to get this treatment funded through an Individual Funding Request.

What To Do What To Do

Ive posted another entry about how creatives heal from walking depression, and here are the highlights:

    • Rest.
    • Develop a meaning practice.
    • Change your life.

These steps are simple to say, not easy to do, so make sure you get as much support as you can.

What To Do When You Feel Nobody Cares

    by Patient Advocate

One of the common laments of someone who suffers from depression is, Nobody cares. When someone says these words it is often very hard to assess the reality of the situation. Is it really true that nobody cares or is this just how we feel at this moment in time?

I have said these words myself during a therapy session. I was feeling extremely depressed and was very gloomy about the prospects of anyone actually being able to help me. More so I questioned anyones motivation to want to help me. This translated into the words, Nobody cares. It was an emotional slap in the face to my therapist, who was desperately trying to help me but I was refusing to listen. It seemed easier to believe that nobody cared so I didnt have to care myself.

I remember the image of my therapist cupping his face in his hands in frustration.

Now I have some perspective on what he may have been thinking. How do you convince someone that you care when they dont feel worthy of love and care in the first place? How do you say I care without it sounding shallow or as a clichéd response? How do you show care to a person who refuses to receive it?

In this post we are going to explore the many meanings of the phrase, Nobody cares and challenge our assumptions in the process. In a follow up post we are also going to take a look at how depression can rob us of our ability to care about ourselves and others and what we can do about it.

    Take Care Of Yourself

When you feel like no one cares…. I Bipolar Barbie

What do you do when no one cares about you? You mustcare about yourself. Exercising and feeding your body the right fuel go a long way to improving your mood and mindset. Take up healthy habits likepriming, meditation, yoga or journaling. Do something you love. Put on your favorite song and sing as loud as you can. Remind yourself that you dont need anyone else to feel happy.

    Use The Power Of Reframing

Meaning is everything and you can learn how to change the meaning you give your experiences, thoughts and emotions. Instead of thinking no one cares about me,practice gratitude for all the things you do have in life. Instead of lettingnegative self-talk take over your mind, practice acknowledging these thoughts and letting them go. Youll begin to notice fewer and fewer negative emotions.

You Feel Like Youre Constantly Faking It

We hear a lot now about imposter syndrome, where people feel that they are just faking it and arent as together as people think. Theres a form of this for those who deal with major depression and other forms of mental illness. You become quite adept at playing yourself, acting the role of the self that people around you expect to see and experience.
