Parkinsons disease is a neurological disorder. It causes motor dysfunction, including tremors, balance issues, and freezing of gait. Freezing of gait occurs when you feel frozen and cant move while walking.

What To Expect On Your First Visit

During the initial visit you will meet with a psychiatrist to discuss possible treatment options offered in this clinic. This usually involves a review of the clinical history including psychiatric symptoms and diagnosis.

Patients will fill out clinical assessment information forms. Most of the first visit will be centered on a discussion of the treatments. We will also speak about the consent process and determine if there is a need for lab work or medicine changes, which may be needed depending on the type of interventional treatment.

If you are a candidate for interventional treatment, you will NOT receive a Ketamine, TMS, VNS or ECT treatment on your first outpatient visit. The nurse coordinator will contact the patient within a week of the first visit to discuss scheduling the treatment.

If you have a patient whom you feel would benefit from Ketamine, TMS, VNS or ECT, we would greatly appreciate the opportunity to meet with and evaluate this person at the Carilion Clinic Interventional Psychiatry Clinic. Please send your referral with a brief summary of the desired treatment and the last progress note including current medicines.

Despite being the second-most prescribed drug, to more than 30 million Americans, antidepressants dont work for nearly a third of those who try them. Annually, more than 40,000 Americans commit suicide, about half of whom suffered from major depression and those numbers are rising at an alarming rate. Suicide is now at the highest level in three decades, with increases in nearly every age category. Emory neurologist Charles Epstein helped refine a novel therapy transcranial magnetic stimulation to become an FDA-approved treatment for depression.What motivated you to work on a treatment for depression?

We had developed an improved transcranial magnetic stimulation coil for another purpose, and learned that teams at the Medical University of South Carolina and the National Institutes of Health were just working out a theory of how TMS might treat depression. Our new coil turned out to be ideal for that application.

It is non-surgical and non-invasive, with very few side effects, and its been shown to work in patients who have not responded to other treatments.

    Major Depression is one of the most common and costly of all psychiatric disorders. While depression can be effectively treated in the majority of patients by either medication or some form of evidence-based psychotherapy, up to 20% of patients fail to respond to standard interventions. For these patients, trial-and-error combinations of multiple medications and electroconvulsive therapy are often required. For patients who remain severely depressed despite these aggressive approaches, new strategies are needed.

    This study will test whether high frequency deep brain stimulation of the subgenual cingulate white matter is a safe and efficacious antidepressant treatment in forty patients with treatment resistant depression, and to investigate potential mechanisms of action of this intervention.
